Medical Bill Question

Thanks to "observer" and "angie" for replying to my credit card limit question. Now, I have another problem: I received a statement from health insurance my husband had at his last job that a claim had been filed with them from almost 3 years ago and of course it was denied (too much time passed). Today, I received the bill from an ER doctor for a visit to the ER in June 2000. I don't have the records of that visit, but I'm sure one ER doc was paid, and I only saw one doc. Conveniently, the doc's practice has dissolved, so I don't have any way to contact them. The bill was sent from an outside agency. I contacted them and they said that I am responsible for the bill because the insurance company did not pay it. Well, they were not even billed for it until now. They said it doesn't matter, that I was my responsibility to call any doctors or facilities that I used to be sure the insurance was billed in a timely manner and to make sure there was nothing more owed afterward. Now, I'm wondering if there is any way the insurance company will back up and pay, or am I stuck with the bill?
