Allen TX 75013 March 10, 2003 Dear Credit Bureau, This letter is a formal complaint that you are reporting inaccurate personal information. I am confused to see how many different variations of my name, address, employers and even my birth date is being reported. This is highly embarrassing to me and I am reporting to you the correct and legal versions of my personal information. I have highlighted in yellow the correct versions of my personal information. The following information therefore needs to be corrected on the report: Correct Name:
Who I am (name here)
I am asking that you delete all of the names other than Who I Am. I am no longer married and have not used the name of Ex Husband in more than ten years.

This is not me (wrong name)
This is spelled wrong (wrong spelling) These are incorrect listings of my birth date:

The correct date of birth is:
01/01/1968. These are incorrect listings of my address:

Apartment Complex I am asking that you delete all of the above addresses for they are out of date or incorrect. It looks as though I have moved twelve times, when in fact I have moved three times in the last thirty years. I am no longer married. I have been divorced for more than ten years now. Please remove the name of the spouse as Jerk Head. It should state none. Telephone numbers: Two of the numbers listed are very old numbers. I would like for you to delete old as well as current phone numbers from my report. I have been getting a lot of calls that are unknown or out of area and I believe it is because of the listing on my credit report. I pay to have an unlisted number, and feel it very unfair for you to list it. Employers:
I have never worked for a company by the name of Some Unheard of Company. Please remove this information. I am currently unemployed, and ask that you also remove Where I Was Working, as I have no plans to return to work for this company. Please remove the above information as quickly as possible. I feel that it will be very harming to any future employers whom might need to see my credit report. This report makes it seems as though I am twelve different people, living in twelve different places having moved twelve different times. Who was born two different years. Sincerely,
