There are NO guarantees with ANY advice I give, however, so far, those who have used the "Letter to HC Providers" CORRECTLY and followed the instructions COMPLETELY have reported on other forums that it has worked.

It is ONLY for medical/hospital collection accounts, NOTHING ELSE.

There is a "follow-up"letter to the CRA's to be used AFTER the first part is COMPLETED. AGAIN only for someone who doesn't decide to "throw in"extra stuff.
or use "additional explanations".

Once you have used this letter, ANY correspondence relating to this account MUST be ONLY to the CRA or the OC- NOT to the CA, as that would negate the whole principle of the notices.

In your case, you would have to do 3 separate letters for the 3 accounts. If one of them should have been billed to your insurance, and it was a hospital billing error, then on THAT letter, you do NOT send a payment, you use "insert b".
