The HIPAA letter is to be used AFTER regular disputes of "not mine, no such account in this amount from this (name of health care provider)on this date" have been sent CRRR, AND not deleted, with a LETTER from the CRA stating that the account has been VERIFIED.

You cannot send the HIPAA letter FIRST withot taking the opportunity to dispute it.You NEED to have it on record that the dispute was made.
