I'd include a copy of the documentation with your letter. I would also send copies to the collection agency as well as the original creditor, and demand an update.

Make sure you keep copies of everything! Having it in writing usually seems to help (though not always).

The re-aging is really starting to get annoying - but keep pushing!

If it becomes really bad and you don't get a satisfactory response, contact your congressional representative - I believe there was some legislation about this type of situation a while back (but I'm not positive). At least they can help you for free (plus, if there's not yet any legislation, they might be able to initiate something)!
