You can use the HIPAA form to pay the OC health provider, however, you will NOT be able to then take any action against the CA for violations.

Once the HIPAA proccess has started, any communication from you to the CA could be considered a waiver of your privacy rights.

A medical or health care provider bill is NOT an "open" account. It is covered under the UCC provisions for goods and services, (4 years),unless you have signed a contract to make payments,in which case it would be a written contract.
