1) Providian. I'd dispute the duplicate accounts. However, are the reports reflecting different account numbers for each one? If it were me I'd start with obtaining all statements for the disputed time periods. There really isn't other way to get around that, even though it generally costs for each one; in my case, it was well worth the cost when I was disputing.

2) Zwicker and Associates. Did you get the agreement in writing, or was it verbal?

3) General question. See if you can dispute these account statuses and request that the credit reports are changed to reflect "closed by consumer" (this worked for me). Generally, accounts closed for non-use shouldn't harm you, unless there are late payments on them. The ones that are still open really aren't hurting anything, but I'd keep an eye on them just the same.
