HIPAA on something that is yours?

Quick question for you (for WhyChat and others) - in a recent post, you had said that the HIPAA letter you provide only works if the account's been PAID or is not yours. But I had posted a while ago about 2 different problems - that a 4-year old medical bill is on my report because the provider never billed my insurance co properly (despite having information). Another provider on another bill didn't bill me properly after I changed address. Does insert B in the HIPAA response letter work here? I was the patient, so the bills are mine, but neither was processed correctly (all other bills involved in the surgery were). Before I send the HIPAA letter (and the relevant California statutes), I wanted to make sure that the letter may work.
