You need a subponea duces tecum. That is a request for production of documents. You specify a location for the other party to bring or mail the documents.

Be very specific about what you want as they will give you exactly what you ask for.

If you can, go to the main courthouse in your city and go to the law library and ask for assistance. They can not give you legal advice but they can help you find the form in a book. Then you copy the form and either fill it in or type it and put in the information you want.

Usually those books also have instructions as to how to use them.
