STOP PAYMENT! If your bank will not issue a permanent stop payment then take all of your money out of this bank and open an account in a DIFFERENT BANK. Also, put your wish to stop payment in writing both to the CA and the bank and send it Certified mail. If the CA continues to charge your account and the bank continues to pay that's their own problem since you no longer have funds in the bank.
