IS CA VIOLATING C&D AGAIN?

A CA holds 3 medical bills owed by us. We have been paying $20 a month on this and have it down to around $1000. They had been harrassing and threatening us if we didn't pay more a month, so a while back we sent a C&D to them (which they violated). We reported them to the FTC, who contacted them. The CA, of course, denied it, but they did stop calling. We have continued to send the $20 a month. The other day, we got a letter from them stating they had received another balance to collect ($43). Since then, they have started calling again, although I haven't spoken to them (caller ID). All of the bills, including the new one, are under the same account with one account number. Do I have to send another C&D for this bill, even though it is all under one account, or are they violating the C&D? Thanks, Tina
