Since you are in DFW and I am in Houston, I would reccomend that you check out O'Connors forms. They will have an answer for a suit on a swron account or a suit on a contract w/ discovery to be derved o the plaintiff.

Check this out at the SMU law library or if you want to spend the money for the forms book, its about $50.00 and available and the SMU law school book store.
