Take your credit report to the police and, file an ID fraud complaint. Also, fill out this fraud affavidate from the FTC.

As Scott suggests, contact the fraud dept of Citi for a mailing address and send this CRRR. Contact the collection agency and send them a copy of the police report and tell them to halt all collections that this is now between you and Citi. Send this CRRR as well.
