Help finding UCC for medical collection

Can someone please help? I am trying to find the Indiana code for medical collection. I have searched through the code and cannot find it. I am sure it is there but I haven't been able to find it. My husband has medical ca on his report that is not only wrong but I believe is past the SOL and I want to be able to give the specific code when I reply to the ca. Any help is appreciated.
