There are two approaches to this. The Investigative method is to plumb the depths of this stopped up toilet to see just what evils lie at its bottom. This takes time, reasoning, formulation of hypotheses and proves quite taxing albeit rewarding.
Another approach which admittedly this unworthy writer is still trying to master is the simplistic approach. In other words throw this pile of dogplop back in their lap and let them pick at it.
In your case you've identified the essence. Forget misfiling, redundancy of claim, and other phony excuses made by the collection swine. It stinks of fraud to you so fraud it is. Write the FTC and your state AG today and tell them of your experience and it's fraudulent appearance.
In this way you control the outcome of the investigation by stating hunch as fact and allowing others to perform the reasoning for you. In other words you get there first with the paper accusations secure in the knowledge that the enemy now must play an unending game of catch-up.
