Previously deleted item re-entered

In applying for a new mortgage, we discovered that a chargeoff that was successfully deleted has been re-entered without notice to me. I am writing each CRA as below with copies of my previous letter and their response showing it as deleted. The mortgage company's credit service is deleting from their end as I provded the documents, but it is impacting our obtaining homeowners insurance. We are in Va, the previous request was 3/15/02 with responses of deletion from each CRA in April '02.

Is this the correct process or letter? Other recommendations? Thanks!!

To whom it may concern;

Through a recent mortgage application process, we have learned of an error on my credit report concerning American Express, account #3785xxxxxxxxxx, amount $3505, shown as in collection.

Please be advised that on March 15, 2002 I contact you regarding this account shown as in error and your response dated April 4, 2002 (copies attached) agreeing and deleting the referenced account. Through this recent mortgage application process, the referenced previously deleted account has shown to be reentered in error, without the required notice.

Please remove the error immediately as it is having a detrimental effect on our mortgage application process and provide an updated report and confirmation showing the deletion.
