I would get all the information you can from the hospital/doctor and contact the insuranc company. yourself. My insurance company only approves a certain amount based upon a schedule/or average cost of service for the area then the hospital/doctor writes off the remaining balance as an insurance adjustment. Sounds like the hospital is trying to get you to pay the "adjustment" they should have taken off. My experience is that paying 3-400 on a 1,400 bill is not unusual. This situation will take a little hard work. As a note my wife is disabled and we have a lot of medical bills and I have to deal with this same type situation all the time....fight!!!! Don't pay something you should not have to.
