Service of blank form

Here's another question.

What about service of a blank form. Before a case number is obtained, before the filing fee is paid, before the even clerk signs or dates the service, can you be served for an impending lawsuit with a blank unsigned form.

The was from a CA lawfirm, the attorney's signature was rubber stamped, and the supposed debtor information was typed in. But all of the court information as in case number, clerk signature and the other things were blank. It seems as if the servor took the form from the CA lawfirm and before filing for a lawsuit did a service.

One more thing, what about this. Someone say I saw this form on the ground. Sorry, it's been a while, but I forgot here you go.

Alas the default judgment occurred.
