There is really no template to use. I went to the courthouse, got the small claims papers, and where it wanted to know what I was suing for, I listed the sections numbers of the FDCPA.
In court is where you explain it.
Or, if you file responses to the other side you explain it.
The filing itself you want to keep as simple as possible.
