Who did you address your HIPAA letter to? Who signed for it? It is SUPPOSED to go to the LEGAL dept. of the HC provider, not their billing dept.

The HIPAA letter was addressed to the legal department of what I *thought* was the HC provider. The original bill was for a doctor in the ER and it turns out the HC provider (the ER doctor) used a billing service. Their business name sounds like it could be a doctor's medical group so that's who I sent the HIPAA letter to. It was signed for by an individual; I don't know if s/he is in the legal department. I suspect they don't even have a legal department.

I do not know if there is another HIPAA violation involved, however, I suggest you send the follow up letter to the CRA, the copy and cover letter to the LEGAL dept. of the HC provider, and include a copy of the original HIPAA letter and a copy of the green card.

I will do that.
