Hard Inquiries

I live in Missouri.
Recently, I was turned down for a credit card due to the # of inquiries on my CR within the past 5 months.
I had 1 bank card inquiry (because I needed one).
2 mortgage inquiries (because I was shopping).
2 Auto insurance inquiries (because I was changing insurance companies to get a better rate).
There was also an auto insurance inquiry that I have little or no knowledge of, but I have an idea.
I "think" that my old agent tried to get me a better rate after I informed him I was leaving his agency.
This, I would consider, unauthorized.
And of the other 2 auto insurance inquiries, both are with the same company and I don't understand why they did 2 hard inquiries.
Is it right that a bank would hold these types of inquiries against me?
Is it possible to get the unauthorized one removed?
Is it possible to get ONE of the TWO removed by the insurance company I now have auto insurance with?
That would bring me down to only FOUR inquiries, instead of the current SIX and possibly look better on my CR.
What do you know about this?
