CC hardship programs

Any advice on requesting a hardship program on my CC. Have a lot of CC debt, don't want to default, but now I am getting caught in the % jackup game. Which companies are easiest to deal with, what works with them. Part of the reason my balances have increased so much is health related and not able to work as much, so plan on including that. So woulc appreciated input from anyone else who has requested hardship or used a hardship program. Thanks so much.
