To prove the "debts" existance you would need the signed charge slips. The only thing an application for credit does is prove that your signature is on a Credit Application and the only thing a Revolving Credit Plan does is prove that a plan exists. Neither the Revolving Credit Plan or the Application for Credit prove the actual "debt". The only thing that proves the "debt" is the signed charge slips. However, I always ask for all three, with emphasis on (signed charge slips) just to make it as difficult on them as possible.
