I do NOT suggest you use ANY form letter (particularly one like "Axiom"'s). You want to keep it simple and low tech to avoid raising any red flags that you know the laws or are VERY concerned about your credit. Keep it personal, something like,
"Dear Mr CA
Please stop calling me about acccount # xxxxxx as I am not allowed to get calls at work and could lose my job, and have a sick old granny at home and need to use the phone for emergencies,and it keeps waking her up. Please just write me, and I will reply." Send it certified mail, make sure you do it yourself,and put the postage on from regular left over stamps, about 20 or so to make up the right amount.
