Can you get to the law library at your main courthouse? They have lots of reference books that have forms for anything you can imagine.

The librarians are very knowledgeable so tell them what you need and they should be able to help you find some books to search.

This is a time when I find using physical books is far better for research than the internet- and faster too.

When I do case research, I prefer the books to the net, esp for shepharidizing.
