some suggestions:

If you can have mail forwarded from your current address, indefinitely into the future, that is best. Otherwise, get a private mailbox, give out that mailbox to all your creditors, and get your mail forwarded every two weeks or so to your new address.

After changing your address, do not let even a snippet of info reach your credit report, no credit checks, no giving out SSN etc.
