Did not validate and now showing on credit report.

I would like to know what I should about this. My husband requested validation of a debt in November 2002. We mailed it certified mail so the CA would have to sign for it. I no longer have the original green card that we got back in the mail but I do have a copy of it front and back. We had not checked his report for some time and went to see about applying for a home loan a few months ago. Guess what showed up? The original amount when we were first contacted was $5366. Then in Oct 2002, the letter from them stated that since we were not interested in trying to work with that with the interest now added, the total is $10,095.72. Also the original loan was taken out in 1994 but my husband refinanced it and the last refinance date was 1997. Now it is showing up under the CA agency name with date opened as 10-02. Any ideas?
