Did I Opt Out correctly?

I went through the standard opt-out procedure from a pay phone so as not to give the CRAs my new number. I have to say the process was a bit confusing and I'm not sure why they asked for some of the information they did. I'm not sure if it worked because the recording said something about sending me some kind of form to verify that I wanted to opt out, yet I never received this form (I opted out about 6 weeks ago). I also recently pulled my CR, and noticed that a "soft inquiry" show from a CRA, due to me requesting a credit report on myself. Not sure if I should try opting-out again.
