If all you need to know is how to request a jury trial, then you can do that in most courts by "indorsement on a pleading", which means at the end of your complaint or answer, put a line like "TRIAL BY JURY IS HEREBY DEMANDED OF ALL ISSUES SO TRIABLE." It's possible to file a separate motion or demand, but sometimes there are time limits so it's best to do it at the first opportunity.

However, I think what you really want/need is probably more than that. Can you fill us in?
