What do I do about this on credit report??

Loan : Open 12/93 Last 10/96 Delinq prior 10/96 Max Delinq 9/97 charge-off
Then it is listed on a collection agency open 10/98 last 8/98 delinq prior 8/98 and the amount is more.
What are the steps I need to take for this ?? Thanks.
