Report these insects to the medical society -assume MD to be your 'former' physician. Report CA to FTC and to your AG also, for fraud.
Send a nice note indicating they've not properly validated according to regulations, further they've illegally inflated the bill.
Whisper sweetly to these pig-ignorant fecaloids that in addition to countersuing for violation of FTC regs, you'll also sue for slander of credit and defamation.

Now, these bugs claim you owe them, what? A hundred bucks? They're going to sue for that? Perhaps. But the counterforce you'll exert against them will result in a much larger amount owed to you. Offer to trade bills, something this worthless writer has done in the past, with fine results.
The avaricice. The avarice just never stops.
