The same thing just happened to me with UNVL/citi. I just read from someone that it is an ATT/Universal card. I called their customer service number (1-800-423-4343) and she told me I had to fax the Credit Bureau Dispute Unit at 605-330-6780 and tell them to call my phone number to discuss it. Just like you, my credit report lists them from years ago and though I have no balance, the account is still open and current.
