This is a long shot, but just maybe.
Threaten to report your doctor to your state medical board of examiners. It could force some action. Anyway, he will have to look at this personally and deal with it. So, the little receptionist, or billing girl in his front office who probably is responsible for all of this will be history. Since this goes back many years, she is probably history anyway. More often than not, front desk personnel, screw up and the doctor himself does not have a clue. What state are you in.
