Medical collections on credit

I have a mark on my credit report that I didn't know about untill the other day. A doctor's bill, that I thought was paid by insurance, has shown up on my credit report ($300.00). It was from about five years ago. Do I dispute it with the CRA, contact the doctor, the insurance company, ??? any advice.
