For accounts that have gone to collection agencies, you list the original creditor on the BK form and then under the OC you list the CA's that are collecting on that same debt.

As said before, LIST EVERY SINGLE DEBT, no matter what its for or to whom. Unsecured debts go on one are, secured debts, like a house or car, go in another section.

If you're actually going to file, STOP PAYING UNSECURED creditors, but keep paying on the SECURED debts.

You must also cease using ALL credit cards for at least 90 days prior to your BK filing date.
