I need some help with National Arbitration Forum Forms

I received a form letter today National Arbitration Forum re: NCO Portfolio Management, Inc., c/o NCO Financial Systems, Inc. I have accessed Why Chat's Arbitration Refusal Letter. This letter consists of four printed pages. Do I complete and retype the first page and conclude the letter through Sincerely, (type-do not sign your name) area? It looks too me like there is a second letter to file if an award is awarded, am I understanding this correctly? Do I then attach the Why Chat's letter to the document from the National Arbitration Forum "Response Form" and fill in where Respondent States: "See attached letter of Refusal of Arbitration"? On page two of the form from National Arbitration Forum, do I ignore all of the information that they want completed regarding type of hearing, etc., Representation Information (I don't have one anyway)? Do I sign: Respondent's Affidavit of Authenticity, or do I leave that blank since I did not sign my name on Why Chat's form and only typed it there? I want to be sure I respond correctly. From what I read in a recent message posted to someone else, it appears that the National Arbitration Forum forms must be sent. My situation is such that I have not yet filed bankruptcy. I had to quit working in September 2001 and continued paying my bills until I exhausted my savings in November 2002. I never answer the telephone, I use call screening, and I have never answered anyone's letters. My source of income is Social Security Disability and a small ERISA pension from my deceased husband. Where I used to work brings work to my home and I make $150. a week from them before taxes are withheld. I believe all of my income is "judgment" proof from what I read several years ago. I do not own a car, it was stolen in August 2001 and I used to proceeds from it to continue paying my bills. I do not own a home. I am the beneficiary in my mother's will to her home approximate value $125,000. with a $45,000. mortgage and her 1989 Toyota. She is 83 and in poor health. I am on her checking/savings account with no co-mingled funds, all of her deposits are from SS and pension, as I pay her bills for her. I have not filed bankruptcy, hoping to ride out the statue of limitations. I am not sure what to do at this point. There are three credit cards that I still pay every month as that is all I can afford. The rest I had to quit paying. I live in the State of California. Whatever suggestions you can give me regarding these above referenced forms will be greatly appreciated. Thank you for your time, Cathy
