If this were an official court document, it would HAVE to be filed with the court and a COPY of the form would go to the attorney's office, certified registered mail.

I think they are blowing smoke. I've never heard of such a thing, and as long as the court didn't tell you to fill it out, I wouldn't if I were you.

Check with the court to see if this is standard procedure. If it isn't a court document or requirement, tell Midland you'll see 'em in court.
