Help- 2 differenct collection agencies same debt

Please help, I have two issues 1) I was looking at my credit report and noticed the following. I have a charged-off debt from Providian for $2070. This agency named Dunstone financial is showing as the collection agency. One tradeline says the account is closed. The other tradeline from the same company says it is open and then there is another company PDI Managment that also has the debt for the same amount listed. What can I do? It seems like my debt is being reported too many times. I can understand the original creditor and maybe a collection agency, but I'm lost. Can anyone give me any insight. 2) there is a debt listed on my credit report for $6819, that I have no clue about, it says the agency has been unable to locate me. I have not clue what should I do.
