heck, i just used a sheet of notebook paper,,put all info at the top---name, SS, etc and listed each i disputed and why. as...not mine...as wrong reported...etc. if it comes back as not verified,. they remove it. if it comes back verified, then i wrote the OC,,which the CRAs will give you all the info on who it is and address.
