i may not be right but i never use forms. its makes the CRAs suspicious. i use plain notebook paper and talk english to them, seems to work good so far. as in,,"this is the same account...only the numbers have changed. you cant do this legally, so take one off." course you stick your name, address etc on it.. they did. lol. doubt they will remove both...the oldest one they should. citi is bad about doin that. at least one must show 0 balance, as should the OC, if it is sold.
