Received a Response from My HIPAA Letter

I sent a HIPAA letter to the Medical OC. I don't think is was even a day after I got the green card back, that I received a response from them. Inside the brown envelope they had a cover letter, a copy of my HIPAA letter, a copy of a "consent for medical treatment", a copy of charges (hospital bill), a copy of our insurance card (front and back) and a copy of "Explanation of Provider Payment" from my insurance company. They say, "I've enclosed some information that may prove valuable towards resolution of this issue (see five enclosures)." But they don't say nothing about removing it from my CR, and it pretty much looks like they aren't budging.

On the consent for medical treatment form, they have highlighted there, "I understand that I am financially responsible to the hospital and physicians for charges not covered by this assignment. I agree to pay, in addition to the other amounts herein provided, all costs and expenses of collection, including reasonable attorney's fees."

I honestly don't recall signing this form in the first place. My doctor's medical office is attached to the hospital and he sent me downstairs to get an X-Ray. The hospital billed me separately for the x-ray instead of my doctor billing me. Anyway, I still don't see how that is a consent for them to give my private medical information to a 3rd party CA.

You know when they give these forms to you, the clerk will sit there and say, "Oh this is just so we can bill your insurance" and then try to rush you along into signing it. Anyway, my question is this:

This whole account has so many red flags to it. (For instance, #1 They billed it to the wrong name.) So I had typed up a two page response to their letter. But I went to WhyChat's Page and looked at the follow-up letter. Should I send out this long response letter I wrote, in which I address all the points they raised and reiterated laws and parts of my intial HIPAA letter? Or should just ignore all that and send the simple follow-up letter that WhyChat recommends?
