You can start with a dispute to the CRA disputing the OC entry as "not mine, no such account for this amount for this date".

Do it on the form they send you with your snail mail report, do it by HAND printed.

Make a copy and send it certified but NOT CRRR.

As to any entry from the CA, wait until you get the OC off your report, then if you are successful, use the same dispute method to dispute the CA.

DO NOT dispute it as "paid".
