Use the forms linked below, deny ALL their claim, in the SOL defense part state this "any accounts I MAY have had at one time, with any similar account # with any OTHER creditors had a last transacton date prior to ----- , making any POSSIBLE claim for ANY such account time-barred"
You can add in your affirmative defense, "This claim is frivolous and I request the Court dismiss it with prejuidice and sanction the claimant for abuse of the Court's proccess"

Make sure you file your answer AND notice of appearance AND proof of service with the Court.

You should file your FTC and State AG and BBB complaints, as well as filing a complaint with your State Bar assoc. Include copies of your filed complaints in your answer.
