Montgomery Wards on credit report

I just saw a copy of my credit report for the first time. There is a reference to an "MWARD/MBGA" revolving account for which the last activity was in 1995. Although my credit score was very high, there was a comment about the large number of revolving accounts. Since I only use two of them, I'm trying to close the rest. I do remember buying some stuff from Montgomery Wards around that time, but I don't remember getting a credit card. How can I close an account with a company that's now bankrupt? The credit report doesn't even give the full account number, so I don't know how I could cancel it even if the company wasn't bankrupt.

By the way, there was also a Transamerica credit card on the report that I KNOW I didn't apply for or receive!
