CRA Disput Not Accepted

I have an old cable bill account that I settled with the original creditor back in May(04). The CA for the OC placed it on my credit report and I disputed it with all three CRA's. Nothing happened except the CA reported it again.

I called the cable company and asked to receive some sort of a statement from them showing the accout paid in full and equipment returned. All they sent me was a screen capture of my account.

Is there a letter available that I can send to the CA and the CRA's with a copy of this screen capture telling them the account is paid and they (the CA) needs to remove the account?
