credit report fix

We declared bankruptcy 2 years ago in may, we tried to get a home equity loan but credit report says we had a collection, never recieved anything in the mail about this. Now we have no way to contact the bank First Bankcard, or the collection agency. Sherman ACQ. This card was included in the chap 13 so no collection should have been reported . Help me clean this up
