You should rewrite it and send a set back to them to force them to prove their case. You need to respond or you may get sanctioned. On the admissions you can deny, if appropriate. It looks like they may not have the documents to prove their case so they are hoping you will just admit you have an account with them and owe the money. That's one reason for requesting cancelled checks. The checks may also go to a potential defense that you paid them in full so they need to find out if it is a defense.

You should say you dispute all charges, you have no recollection of signing any agreement with them.

Dp you actually have an account with them? If not, then tell them that. If you are not the right person they sued, then tell them that too.
