Followup SOL letter questions

Morning folks, In my original message (link below) I had a few questions that got me started and then some followups that were taken care of, now I just need three more things clarified.
This is based on a Citibank Visa from 13 years ago.
In the SOL letter on whychats site it states:
Include the following underlined paragraphs for credit card accounts
It then follows with a number of underlined pieces of information. This should all be included correct?
It then states:
Records indicate that this date on the subject account #__ with CITIBANK VISA is __ ___ ____.
I haven't any idea what this date is. How would I ever find this out or is it necessary to include this date?
Finally it states:
Under the laws of my State, (statute # if available) continued collection activities, including reporting, etc
Where would I find this statue if it exists?
