Opt Out Follow-Up Ltr.

Hi! I opted out last week and rec'd a ltr in the mail today "in response to my request..." It references all 4 CRA's and it directs me to check a box if my name & address is correct or check another and make updates below. They are also asking for previous street address, SSN, signature and date. Asking me to mail back to Opt-out depart. in Seattle. It also states: "The personal info that you provide is confidential & will be used only to process your request. Failure to provide complete & accurate info may prevent or delay the processing of your request."
Is this ltr part of the opt-out process? Should I mail it back? Thanks!
