Small Claims Court Help Needed

Providian Financial sued me after nearly five years of disputing unauthorized charges processed on my cc back in 1999. I went to a pre trial hearing last week and disputed the claim with the intention to go to trial and defend my rights under the fair credit billing act. Unfortunately after seeking legal counsel I've had to come to the realization that the most cost and effective way to handle this is to settle out of court. I have made a settlement offer over the phone to the CA's attorney and they have asked me to put in writting, can anyone tell me where online can I find a format for a settlement letter and should I also send a copy to the judge presiding over the case?
